UNDERSTANDING HOSPICE CARE
LEVELS OF CARE
ROUTINE
Available at the patient's place of residence. The hospice team will provide care in the comfort of your own environment depending on your need.
CONTINUOUS
When medically necessary, care is provided in the home (or another appropriate setting) up to 24 hours per day to manage acute symptoms.
Requires constant monitoring in the home, nursing home or assisted living facility.
INPATIENT
Hospice care is provided in a designated healthcare facility for a short period of time when your loved one's medical needs cannot be managed in the home environment.
RESPITE CARE
Patients being cared for at home are offered a brief stay at an inpatient setting to allow family members an opportunity for travel or self-care.
YOU ARE NOT ALONE.
We aim to provide support to the caregivers and families of our patients, as well as the patients themselves.

What is hospice?

Hospice care is specialized care that focuses on the quality of life of patients who are experiencing an advanced, life-limiting illness. We provide compassionate care in the last phases of an incurable disease so that individuals may live as fully and comfortably as possible.
What happens in hospice?

Include the patient and their family/caregiver in the plan of care, we work to relieve pain and other symptoms, assist the patient in meeting individual goals, help the patient and family develop effective coping mechanisms, provide an atmosphere of comfort and compassion while helping patients, their families and loved ones deal with the emotion impact of death.
Who qualifies for hospice?

Individuals with a prognosis of six months or less if their disease runs its natural course as determined by a physician.
Who pays for hospice?

Hospice care is private pay, however we do accept Medicare and Medicare replacement plans. The Medicare Hospice Benefit is an inclusive benefit in which all services that are related to the terminal illness are covered up to 100% by Medicare Part A.
